A key part of modern general practice is the management of chronic diseases such as asthma, hypertension (high blood pressure) and type 2 diabetes. Priory Medical Group has highly trained specialist nurses and pharmacists in all these areas of chronic disease management.
We offer innovative solutions such as asthma reviews via text or video consultation. If you have a question about your chronic disease you can use the prioryCARE online form to get in touch, choose nurse appointment, then “I have a question about.....”.
If you are due a blood pressure check; many of our reception areas now have a “BP pod” and you can book an appointment slot to use this via the NHSapp and your SystmOnline account. You can also do home blood pressure readings and either drop off a paper copy of the results (with name and date of birth included) to any of our reception desks or use the prioryCARE online system and choose the general enquiries tile.
To arrange a blood test - PMG has slots available during morning clinics each week across a variety of our surgery sites. We try to prioritise these for our frail and vulnerable patients who are unable or struggle to attend the hospital run drop in clinics or the CDC. These are pre-bookable up to 2 weeks in advance. You can get in touch via the prioryCARE form (use the ‘nurse appointment’ tile), come in to speak to a reception team member or call 01904 404100. You can collect a blood form, or drop in a urine sample if requested at any of our 8 open surgery sites. Please avoid 12:15 - 13:00 when our reception areas are closed for the lunch break. The blood pressure pod is available at our reception sites in Tang Hall, Heworth Green, Victoria Way, Fulford Surgery, Rawcliffe Surgery, Lavender Grove and Priory Medical Centre.
